Introducing Health Passport: Your Health History, Organised and Ready

If you’ve ever walked into an appointment and felt your mind go blank, you’re not alone.
Medications, past conditions, family history, sleep, stress: it’s a lot to recall on the spot, and it’s even harder when you’re repeating it all to a new provider.
That’s why we built Health Passport. It’s a guided experience that helps you build a structured record of your health once, so you can stop starting from scratch every time.
What is Health Passport?
Health Passport is a patient-authored health summary, designed in a format clinicians can quickly understand and use. Rather than filling out a long form in one sitting, you move through five short conversations, each focused on a different part of your wellbeing.
Our clinical team helped shape every question and the overall structure, so what you share is organised, meaningful and ready to support better conversations with your healthcare providers.
The five conversations
Each conversation explores a different dimension of your health:
Diet and nutrition: how you eat and what your day-to-day looks like.
Sleep and exercise: your rest, movement and routines.
Medical history: conditions, medications and past care.
Risk and sexual health: screenings and the factors that matter for your long-term health.
Mental health: how you’re feeling and what support looks like for you.
You can take them one at a time, at your own pace. By the end, Ovum brings everything together into a clinical-quality summary.
How to use your Health Passport
1. Find it. Open the Ovum app and scroll to Health Passport on your home page. Tap View to open your passport.
2. Fill it in. Tap Start chat to complete the conversations. If anything was missed during onboarding, this is where you can fill the gaps.
3. Review and update. Look over each section of your passport to make sure it reflects you. Your health changes over time, so tap Edit whenever something needs updating.
4. Share it. Before an appointment, tap Share as PDF to download or send your passport to yourself or your provider. You can also simply show the card on your phone.
Why it matters
Good care depends on good communication. When your history is clear and organised, appointments can focus less on gathering background and more on what you actually came in for. Health Passport is designed to help you walk into the room prepared, informed and confident in your own story.
It’s one more way to be your own best health advocate, and you don’t have to do it from memory.
